DS-82 Application
To renew your passport by mail, first confirm that your current document is undamaged, was originally issued when you were at least 16 years old and is not more than 15 years old; once verified, simply fill out Form DS-82, gather your old passport along with any necessary proof of a name change, if applicable and complete the process by mailing your application from your closest UPS Store.
